Important Information for Exchange Students

There are some very important forms and tasks that you need to complete before you arrive at Pitzer for the Fall. Below is information on some of the to-do items – please make sure to read carefully and submit all necessary forms by their deadlines.

 

ACADEMIC ORIENTATION FOR EXCHANGES – August 14 – 16, 2021 

   Group Orientation – August 14, 10:00am PDT 

   Individual 1 hour- long Class registration – August 15 and 16  

         (schedule to be agreed at the Group Orientation) 

   Zoom information will be coming from Kebokile Dengu-Zvobgo, Associate Dean of International Programs

HEALTHY PITZER 

If you have been vaccinated for COVID-19, please upload your vaccination information through the Healthy Pitzer app: Healthy Pitzer 

If your vaccination information is in a language other than English, please provide a translation to help our healthcare staff in assessing your information. 

You will need to have set-up your Pitzer email and login in order to do this. 

 

ORIENTATION REGISTRATION AND AIRPORT PICKUP FORM 

Please fill out and submit this form by August 9 in order to request an airport pick-up.

EARLY ARRIVAL HOUSING FORM 

Please fill out and submit an Early Arrival Housing form as soon as possible through the housing website:

All exchange students need to fill this out, as you will be arriving before the general date for returning students.

PHOTO SUBMISSION FOR PITZER ID CARD 

Submit your photo for your ID card!  Photos must be submitted on time or you will not have an ID card when you arrive on campus in August to access your residence hall or the dining hall. Deadline is August 9.

Digital picture requirements:  

  • File size: 1024×768 pixels or above, with a resolution of no less than 75 dpi.  
  • Head shots only. The photo is for identification purposes. Photos containing hats, sunglasses, props or silly faces WILL NOT be accepted. 
  • The photo should be taken from a distance of about 5 feet, against a plain background (a white wall) in a well lit area, with the digital camera set to take a 1024×768 pixels size image, and with auto-flash on. This photo provides exactly the kind of source material we need to produce a typical – and effective – photo ID card! 
  • Save the photo image in a .jpg or .jpeg format. 
  • Name your jpeg file with your first and last name so we can identify it.

 

MEDICAL INSURANCE INFORMATION 

Medical insurance is mandatory for all students. All students will be automatically enrolled in the Claremont Colleges’ Student Health Insurance Plan (SHIP). To be eligible to waive out of SHIP, you must be currently enrolled in a health insurance plan that meets your school’s waiver requirements. Waiver requirements include being enrolled in a health insurance plan that meets the minimum essential benefits of the Affordable Care Act (ACA), requires you to have access to providers near campus and to have coverage for services beyond urgent and emergency services.

Please contact the Office of Student Affairs 909.607.2821 or email [email protected] if you believe you qualify to waive out of the plan. The deadline to waive out is September 3, 2021. 

Students on exchange through ISEP have comparable insurance through ISEP. Please make sure you contact Katie Tewell before the deadline in order to waive.   

Students on direct exchange must provide information about their insurance plan to Katie Tewell in the Office of Student Affairs in order to request a waiver.

STUDENT HEALTH AND ENTRANCE PHYSICAL 

Your Entrance Physical & Immunization Recordsshould be filled out and submitted directly to Claremont University Consortium (CUC) Student Health Services. Please review all the information thoroughly before submitting the completed form to CUC Student Health Services. You may choose to submit your completed form by mail or email to [email protected]. 

Please take a moment to visit the Student Health Services website as it contains important information such as the SHS forms including the entrance physical forms, psychiatric medication policy, and vaccine information. Although the Student Health Center is closed for the summer, you may access additional information regarding Student Health Services by visiting their website or calling and leaving a voicemail message at 909.621.8222 which is checked weekly. 

If you are unable to complete your entrance physical before arriving at Pitzer, please make arrangements to do so at Student Health Services at the beginning of the semester.
