Voluntary Vaccine Reporting through Healthy Pitzer
Logon to Healthy Pitzer and follow these instructions:
Click on the “COVID-19 VaccineSelf Reporting” link on the main Healthy Pitzer page after you have logged in.
Choose whether you’re submitting a booster or a vaccination.
Choose your vaccine type
Use the calendar option to choose dates for both your first dose, and second dose (if applicable)
*Note: For boosters, please use the ‘Date of First Dose’ calendar option to report the date of your shot.
Click on the gray square area and choose your proof of vaccination file to upload it
Tip: Take pictures of your vaccination card with your phone
For boosters, click the name of your file to upload a booster card.
Click Submit
